Membership to NEVAT is open to all who apply and submit the joining and membership fees. People who require chemotherapy, renal or burns treatment are not required to be members to qualify for NEVAT services.
Membership is due for renewal on the 1st of July each year. New members joining on or after 1st April are considered to have paid for the next financial year. Membership fees are set out as follows:

Membership forms are available using the links below. These are in PDF format and you will need Adobe Reader 5 or higher to be able to open them. These can be printed out, filled in and mailed back to NEVAT, whereupon payment is verified, your membership will be granted or renewed as applicable.
